Jazz-Mijn Fernandez, 23, of Leitchfield, Kentucky, formerly of Rockport, passed away on Thursday, September 15, 2022, at Norton Audubon Hospital in Louisville, Kentucky, surrounded by family.
Jazz was born in Pasig, Philippines on May 7, 1999.
She was very active in her church and with the children’s church program. She also worked at Box 2 Radio as the technical producer.
Jazz was preceded in death by her grandfather, Armingo Fernandez.
Jazz is survived by her grandmother, Salvacion Fernandez; parents, Whitney and Mercy Ward; brothers, Whitney Ward and Wilson Ward; sisters, Abby Ward and Jamie Ward; many extended family and close friends.
Services are 2 p.m. Wednesday, September 21, at Boultinghouse Funeral Home in Rockport. Burial will follow in Sunset Hill Cemetery.
Visitation is from 2 until 8 p.m. Tuesday, September 20, at the funeral home.
